1. Can I pay with cash or personal check? No. Only credit or debit cards are accepted for the DUI Class Online.
2. Do I have to complete all of the sessions? Yes, and in sequence.
3. What if I can’t finish in the 30-day time period. You may request a 30-day extension, but you will be required to pay the $150.00 extension fee to continue. To obtain an extension, click on the extension button below.
5. Can I register for the Victim Impact Panel (VIP) through Intervention? No.Cook County,Illinoisresidents must contact Central States Institute of Addiction(CSIA) to register for the VIP.
6. How long will my records be kept? Intervention is required to keep your files for five years, before being destroyed. You will receive completion documentation at the end of your course. Only current client files are kept at our Central Office location. An appointment must be scheduled if you wish to pick up your paperwork. Completion paperwork can also be mailed or faxed to you. If you completed your class beforeJuly 1, 2003, please allow up 30 to 45 days to process your request for copies of completion papers.
All clients must have the following information to get a file from CSIA:
Social Security number,
Intervention Instruction number (if you took a class before July 2003),
DUI Class Online Client ID and PIN (if you took the online class),
Driver’s license number,
the district the file belongs to, and
your assessment date (found on your "Client Intake Inquiry Sheet").
There is a $10 fee for additional copies of completion paperwork.
7. Do I have to supply a copy of my evaluation? YES, if you received your DUI inIllinois. A duplicate copy faxed to 773.404.9750 is fine. You may keep the original. Your evaluation must be received by Intervention BEFORE your CD-ROM class packet will be mailed to you. If you received your DUI elsewhere, you must provide verification that you have confirmed with your individual court monitor that DUI Class Online will satisfy the requirements of that state’s court order.
